HOW IS RETINAL RUPTURE AND DETACHMENT TREATED?

VISION LOSS CAN OCCUR IF THE RETINAL TEAR IS NOT TREATED
The retina is the light-sensitive tissue that consists of nerves at the back of our eyes. Retinal detachment is a serious eye disease in which the intact retinal layer separates from the inner surface of the eyeball to which it is attached. The cause of the detachment may be a retinal tear. If the retinal tear is not repaired, the problem can spread to the entire retina and vision may decrease. Retinal detachment can cause permanent vision loss if left untreated. In case of “detachment”, retinal surgery is performed urgently. Retinal tears are manifested by visual signs such as flashburst, decreased vision, partial loss of vision, broken body vision, and symptoms such as seeing objects that are larger or smaller than they really are, flashing lights, black dots, or fly flying.
Knowing the symptoms of retinal tears and retinal detachment diseases and getting timely medical treatment, that is, early detection, treatment of the disease and prevention of possible blindness, are of great importance. If the retinal layer is not separated from where it is located, only in the form of a tear, it may be preferable to use an argon laser to surround the perimeter of the tear in the form of a barrier with a simple laser intervention.

CAUSES OF RETINAL TEARS AND RETINAL DETACHMENT DISEASES
In myopic patients with a high number, the risk of retinal rupture is quite high, because the retinas of these patients are thinner than usual. In older people, retinal tears can occur due to disruption of the structure of the fluid in the eye, pulling the retina and tearing it off. After these tears, the retina gradually separates from the eyeball to which it is attached. Sometimes the cause of retinal detachment is unknown and no measures can be taken to prevent detachment. That is why early detection is so important.

CAUSES OF RETINAL DETACHMENT
– The presence of myopia higher than number 6,
– Eye injury,
– Undergoing eye surgery (Cataract, etc),
– Decrease in the fluid layer in the eye with advancing age,
– Genetic factors.

HOW IS THE DIAGNOSIS MADE?
The ophthalmologist drops eye drops that dilate the pupils for the diagnosis of the disease and uses special lenses and microscopes to examine the retina. Thus, it can detect a rupture or detachment. In the diagnosis of retinal diseases, the space created by the pupil can be easily controlled because it can be used to fully observe the inside of the eye. If the attending physician deems it necessary, eye tomography and eye angiography methods can also be used to diagnose the disease.

THE METHOD OF TREATMENT IS DETERMINED BY THE DAMAGE TO THE EYE
The method of treatment in Turkey is selected depending on the degree of retinal damage. Retinal surgery is performed under general or local anesthesia under operating room conditions. Treatment of retinal detachment should be carried out by surgical method. The gel that forms the retinal tissue is put in place. Therefore, the vitrectomy method is preferred for 90 percent of patients. Depending on the progression of the detachment, gas may be given to the eyes. If there is only a tear in the retina, the tear laser is added to the treatment.

WHAT ARE THE PREFERRED TREATMENT METHODS?
Argon laser: It is preferred for the treatment of retinal tears. Patients who underwent microsurgery can be discharged after a day.
Vitrectomy method: Used to repair large tears in the retina. The attending physician will inject a gel-like liquid by removing the transparent gel-like tissue (vitreous) that fills the inside of the eyeball. Depending on the size of the tear, laser and gas or gel buffering methods can also be used.
Scleral smashing method: In this method, an external application is made to the eye. According to the situation in some eyes, vitrectomy can be done to support surgery. It is placed under the eye muscles with a thin strip made of silicone so that muscle remains under the conjunctiva layer covering the white part of the eye. In this way, pressure is applied to the eye from the outside and the retina is aimed to calm down.
Method of gas injection into the eye: It is recommended to do it at an early stage in the eyes with ruptured retinal detachment. Gas is injected into the eye. It is aimed to place the retina in its place by giving the patient rest in the appropriate position. After this procedure, argon laser therapy gives more effective results.
