This is a type of anamnesis that covers all aspects of oral health, including previous assessments and treatments related to oral health.

History of present illness

The patient present in discussions with the onset, duration, characteristics, symptoms contact or relieving factors are obtained. The patient is asked what is the cause of the symptoms and whether a similar condition has occurred in the past.

A THOROUGH EXAMINATION IS REQUIRED

Waiting to get an anamnesis is also one of the most important parts of a patient’s examination. Anamnesis is not only a way to show a wise approach to diagnosis, but also is necessary for the establishment of a successful patient-dentist relationship. In order to establish an accurate clinical diagnosis, a careful anamnesis is required, as well as a thorough examination. The collection of all data from the patient is an essential element in the diagnosis of the disease, the planning and implementation of treatment. In most dental patients, perhaps it may not be necessary to have a complete anamnesis, but in some patients, the patient’s life may be in danger during dental treatment if a medical anamnesis is not taken. When the patient is not allowed to explain his complaints, some of the diseases he has may be overlooked.

A diagnostic interview is an exchange of information that deciphers information about the patient’s health and is conducted between the patient and the dentist. The dentist can understand the nature of the patient’s illness and his attitude to it with the information he will receive from the interview. Every dentist should develop an interview style in order to conduct an effective interview. Otherwise, if the patient retains control during the interview and wastes time telling about topics that he does not contribute to, he will not achieve the desired goal.

DIRECT CONVERSATION WITH THE PATIENT

Some patients do not fully describe their anamnesis. Some of them do not adequately answer the dentist’s questions. If decisions about dental treatment are made based on incomplete or incorrect patient anamnesis, the risk of side effects increases. Therefore, dental care should be postponed until reliable information is obtained.

It is a good practice to inform the patient about what to do and the relevance of the information to be obtained to dental treatment before starting to take a medical anamnesis.

Sometimes it may be necessary for us dentists to contact his doctor so that he can find out the complete medical history of the patient.

During the interview, the patient is regularly asked questions about his health. The patient should be allowed to fully discuss the problems. It is important that the patient’s speech is listened to without interruption. If it is possible that the patient’s speech will not be interrupted, he will often tell about his problems on his own. To make sure that the patient understands what is being said, he is asked to repeat some instructions.

The main method of obtaining an anamnesis is a direct interview with the patient. In a direct conversation, we dentists ask the patient questions on a regular basis. Other methods have been developed in order to shorten the time required for the interview by 2/6. The printed question list and the printed information checklist are examples of these methods. Although it is a good starting point to have the patient fill out the list of questions related to the anamnesis, it may not be enough. This is especially the case when the terms medical and dental are used. Also, the patient may not be able to fully understand all aspects of a problem. In this way, he may not respond in accordance with the truth. Therefore, after the patient has completed the list of questions, this list should be discussed with the patient. This discussion gives the dentist the opportunity to clarify misinterpretations and incorrect answers.

Positive situations should be recorded in the patient’s records. In addition, some negative situations also need to be recorded. For example, The patient reports that he has no known allergies to medications, has no acute joint rheumatism, heart disease and lung problems etc.

ANSWERS TO WRITTEN QUESTIONS ARE REQUESTED

There are advantages and disadvantages to using a written question list. The advantages of this method include:

1- It takes less time than the dentist.

2- It provides a standard approach to each patient.

3- It allows the patient to remember the medications he uses and the various signs that indicate his illness. It also helps to identify areas that need to be asked in detail during the dialogue.

4- A list of questions that are filled in by the patient in a confidential way can eliminate the state of distress that may arise during answering questions about habits, substance use, sexual preferences, or venereal diseases.

The disadvantages are as follows:

1- The probability of establishing a close relationship in the patient and the dentist is small.

2- The questions on the list may be misinterpreted by some patients.

3- In order for the questions to be understandable, they must be long.

4- In order to obtain specific information from some cases, it may be necessary to provide them with a list of other questions.
